This simple smart survey form was designed with the aid of ArcGIS Survey123 to enable the stakeholders in the network operations department, track the maintenance status of distribution transformers. This means, a change in transformer status such as being out of circuit, in circuit, loaded or de-loaded (with or without customers connected), metered or unmetered, etc., can easily be reported via this app. It eliminates the traditional manual process of reporting network changes.

The date and time of occurred changes alongside name of the field officer who makes a submission is known. This gives room for accountability.

The location where the submission is made is also known, which makes the credibility of the submitted data entry verifiable.

Report on the data collected via the app, can be viewed immediately on the ArcGIS Survey123 web designer. Data collected can be exported in different formats and shared for further processing and analysis to make informed decisions.
